Boy Scouts of America Annual Health and Medical Record information.

All Scouts must have Part A, B, and C completed on the Boy Scouts of America Medical Form:

Forms are valid for 1 year.

For High Adventure Scouts, Part D is requited to be given to the doctor during the physical.

All Scouts must have a "Parent Guardian Permission Consent Form.pdf" on file.  Please print this form from below.
